Q) What does an endobiogenic evaluation entail?

Q) I would like to schedule an appointment. How do I proceed?
        ● Dr. Jean Claude Lapraz comes from Paris, France to the USA 3-4 times per year to see patients at EIMC. To schedule an appointment with him, please call 208-478-8400 (toll free 877-470-8400) or e-mail belen@eimcenter.com. Belen will also arrange your LabCorp draw for you.

        ● Dr. Laramie Wheeler is currently seeing patients at Blackfoot Medical Center every Monday and Thursday. Please call 208-785-2600 to schedule your appointment. The specialized exam required for an endobiogenic evaluation takes additional time. When you call Blackfoot Medical, please mention that you are an Endobiogénie patient, and you will be given a longer appointment. You also need to call EIMC 2 weeks before your appointment with Dr. Wheeler to arrange your LabCorp draw and to pay for your endobiogenic evaluation. Once your labs are received, your Biology of Functions will be processed and the results sent to Dr. Wheeler. After your exam, our Endobiogénie team will form a treatment plan for you. You will have a 10-20 minute follow-up appointment with Annette Davis, C.N. (at EIMC or by phone) to review your Biology of Functions results, treatment plan and answer any questions you may have.

        ● Dr. Jean Bokelmann is currently on a leave of absence. An update will be posted when she returns.

Q) I am not able to travel to the clinic at this time. Can I have a distance evaluation?
    A)  Yes. While a face-to-face visit is preferred, we understand that travel is not always medically feasible. Before your labs are drawn, you will be required to sign a declaration of informed consent which includes authorization for us to send critical information such as lab alerts to your primary care provider. We highly encourage a face-to-face visit as soon you are able.
 
Q) Is Dr. Wheeler an Idaho licensed physician?
     A) Yes. Dr. Wheeler is a D.O. licensed by the Idaho Board of Medicine.

Q) Is Dr. Bokelmann an Idaho licensed physician?
     A) Yes. Dr. Bokelmann is an M.D. licensed by the Idaho Board of Medicine.

Q) Is Dr. Lapraz an Idaho licensed Medical Doctor?
     A) Dr. Lapraz, the co-innovator of Endobiogénie, is a French licensed Medical Doctor. He comes to the USA from Paris, France as a guest consultant to assist with your selection of herbs and supplements, and to recommend dietary and lifestyle modifications only. Dr. Lapraz is not an Idaho licensed M.D. or D.O. and he is not licensed under the provisions of Idaho Statutes 54-1804.

Q) What do I need to bring with me to my appointment?
        A) Please bring (or send in advance) the following:
                1) A copy of your insurance card (if applicable).
                2) Choose an appropriate health history from the list below:
                         First time Endobiogénie Patients:  Health History Questionnaire
                         Follow up Endobiogénie Patients:  Follow Up Health History Questionnaire
                         Children ages 0-7:  Pediatric Health History, Ages 0-7
                         Children ages 8-18:  Pediatric Health History, Ages 8-18
                         Women's Health Exams:  Women's Annual (Preventive Health) Exam Questionnaire
                3) A copy of any pertinent medical records: Records Release
                4) Choose the appropriate Document of Understand from the list below:
                            Document of Understanding for Dr. Lapraz
                            Document of Understanding for Annette Davis, C.N.
                            Document of Understanding for Dr. Bokelmann
                            (No document of understanding is needed for Dr. Wheeler since she is able to act as your primary care physician if desired).

Q) What labs are required?
    A) Most patients who desire an endobiogenic evaluation require a Rank 3 labs.  The charge for this extensive lab work (over $500.00 value!) is included in your endobiogenic evaluation fee. We also include PSA for men 40 and over at no additional charge.

Q) When should I have my labs run?
       A) Please have your labs drawn at least 2 weeks before your appointment so your that your Biology of Functions report can be ready for review. Note: A 12 hour fast is required prior to your lab draw (you may drink water and take any required medications).

Q) Where can I go to have my labs run?
      A) Once payment is received, we will fax an order to LabCorp:

If you live out of state, a lab order can also be faxed to your local LabCorp. Please see  http://www.labcorp.com/psc/index.html to locate the LabCorp draw center nearest you. Call or e-mail us with the phone number of the draw center of your choice, provide us with your payment information, and we will fax your lab order.

Q) Can I be seen at EIMC without Biology of Functions labs?
    A) Yes. While we consider the Biology of Functions to be a very useful diagnostic tool, it is not an absolute requirement. We also offer a "basic evaluation" which looks for endocrine signs based on your history, symptoms, and physical exam.

Q) If I have questions regarding my herbs, supplements, dietary or lifestyle recommendations, who can I ask?
     A) Annette Davis is our Certified Clinical Nutritionist and Endobiogenic Practitioner. She has been studying Clinical Phytotherapy & Endobiogénie with Dr. Lapraz for 20 years, and will be able to answer questions about your herbs, supplements, dietary and lifestyle recommendations. We remind you to consult your primary care physician for pharmaceutical needs, ongoing health maintenance, prevention, evaluation and management of any acute minor or serious medical problems.

    A) Yes. Below is some information to help you with airline and hotel reservations.

    A) Yes, we accept Visa, Mastercard and American Express.

Driving directions: From I15, exit Clark Street and head West on Clark Street about 2 miles until you go under the underpass where Clark Street turns into Center Street. We are located upstairs in the Valentine Building (large white building on the corner of Center and Arthur in Old Town Pocatello). Please check in at Ste 101 (Time Laboratories) downstairs.

Routine gynecological care is essential for all women. Women's preventive health exams are now offered by our female providers in a quiet, comfortable, relaxed environment.

It is important for patients to understand that although Endobiogénie and Biology of Functions have been studied closely and extensively in France, they have not yet been subjected to the requirements of scientific scrutiny according to American medical standards. However, since herbs and supplements are available over-the-counter to all Americans, it is our belief that patients are serving themselves well by undergoing an endobiogenic evaluation to assist them in selecting herb and supplement therapy with the greatest potential benefit and the least potential risk to them as unique individuals.
