What are the costs?
Physical Exam (billable to most
insurance carriers)
Your physical exam will be performed by Jean
Bokelmann M.D., Idaho State University faculty member in
cooperation
with Idaho State University Integrative Health Clinic. The Physician's
fee, which is a separate charge from your Endobiogenic Evaluation can
be billed to most insurance carriers upon request (sorry, we cannot
bill Medicare or Medicaid). Applicable patient co-pays and deductibles
are due at the time of service. Any portion of the fee not paid by your
insurance carrier is your responsibility. The Physician’s fee is
subject to a cash discount if you pay in full at the time of
service. (If you
decide to take the cash discount, EIMC will not be able to bill the
charge to your insurance provider).
