What are the costs?

Physical Exam (billable to most insurance carriers)
    Your physical exam will be performed by Jean Bokelmann M.D., Idaho State University faculty member in cooperation with Idaho State University Integrative Health Clinic. The Physician's fee, which is a separate charge from your Endobiogenic Evaluation can be billed to most insurance carriers upon request (sorry, we cannot bill Medicare or Medicaid). Applicable patient co-pays and deductibles are due at the time of service. Any portion of the fee not paid by your insurance carrier is your responsibility. The Physician’s fee is subject to a cash discount if you pay in full at the time of service. (If you decide to take the cash discount, EIMC will not be able to bill the charge to your insurance provider). 

NEW! Women's Annual (Preventive Health) Exams
    EIMC now offers Women's Health Exams performed by Jean Bokelmann M.D., Tina Mladenka MSN, OGNP, RNc, or Virginia Murphy RN, MSN, FNP-C. The fee can be billed to most insurance carriers upon request (sorry, we cannot bill Medicare or Medicaid). Applicable patient co-pays and deductibles are due at the time of service. Any portion of the fee not paid by your insurance carrier is your responsibility.

Optional Endobiogenic Evaluation (out of pocket charge*)
    Level 1: $300.00. Preventive care and patients with non-degenerative pathologies, seeing Dr. Bokelmann. This fee includes the following:
    • Extensive lab work performed at LabCorp including: CBC, TSH, LDH, CPK, Osteocalcin, Alkaline Phosphatase with Hepatic, Bone, Intestinal Isoenzymes, ESR, Lipid Panel. (If applicable, any additional labwork ordered will incur a separate fee.)
    • Processing your lab results through the Biology of Functions software in France (summary printout included).
    • Recommendations for supplements, dietary and lifestyle changes by our endobiogénie team. 
    • The physician's fee is NOT included (see above).
    • A follow up clinic visit is recommended at 3 months (to review your follow up health history questionnaire; labs are generally not required for the 3 month follow up visit).
    • Follow up distance evaluation/interim lab review: $375.00 (see our FAQ page for more information)
 
    Level  2: $500.00. Patients with degenerative pathologies including cancer, seeing Dr. Bokelmann for their preliminary consultation (a follow up appointment with Dr. Lapraz is recommended).
    • Processing your lab results through the Biology of Functions software in France (summary printout included).
    • Recommendations for supplements, dietary and lifestyle changes by our endobiogénie team. 
    • The physician's fee is NOT included (see above).
    • Note that specialty lab work is required for cancer patients and the charge is NOT included in the fee. 

    Level 3: $600.00. Personal consultations with Dr. Lapraz (except for cancer patients, see level 4 below). This fee includes the following:
    • Extensive lab work performed at LabCorp including: CBC, TSH, LDH, CPK, Osteocalcin, Alkaline Phosphatase with Hepatic, Bone, Intestinal Isoenzymes, ESR, Lipid Panel.
    • Processing your lab results through the Biology of Functions software in France (summary printout included).
    • Recommendations for supplements, dietary and lifestyle changes by our endobiogénie team. 

    Level 4: $1,000.00. First time cancer patients seeing Dr. Lapraz. This fee includes the following:
    • Personal appointment with Dr. Lapraz.
    • Processing your lab results through the Biology of Functions software in France (summary printout included).
    • Recommendations for supplements, dietary and lifestyle changes by our endobiogénie team. 
    • Follow up visits are $700.00
    • Note that specialty lab work is required for cancer patients and the charge is NOT included in the fee. 

* Please note that the Endobiogenic Evaluation is billed as a separate charge from the physical exam. It is an out of pocket charge that cannot be billed to your insurance provider. Half of the fee is due in advance when you schedule your appointment and the balance is due the day of your appointment. The half down portion of your fee is non-refundable, even if you cancel your appointment. As a courtesy to those patients on our waiting list, please provide at least one week notice if you need to change or cancel your appointment. Fees are subject to change at any time.

➩  If you are a local patient, your labs will be ordered at your initial visit and then reviewed with you at your follow-up visit approximately 2 weeks later. If you are coming in from out of the area or are seeing Dr. Lapraz, please have your labs drawn 2 weeks before your appointment so that your Biology of Functions report can be ready to review with you during your appointment. A 12 hour fast is required prior to your draw (you can drink water).

Herbs & Supplements 
    EIMC does not sell supplements;  however, we will refer patients to an appropriate lab to obtain them. Most insurance providers do not cover the cost of supplements.
